Elevate (ANSA EAP) — Privacy Collection Notice (v2)
We collect your name, work email, mobile, company code, and (with your consent) mental-health screening responses, daily mood check-ins, and practitioner-booking information. We process this information to deliver the Elevate well-being platform and, where the law requires, to trigger a safety pathway.
Who sees what (third-party disclosures)
- Halaxy (AU) — your name, email, and booking details, for appointment scheduling with our partner clinicians.
- Resend (US) — transactional emails such as welcome, password reset, OTP, and appointment reminders, delivered via Amazon SES (Asia Pacific, Tokyo).
- TouchSMS (AU) — one-time codes and SMS notifications.
- Apple APNs and Google FCM (US) — push notification delivery. We send only a non-clinical placeholder body ("You have a new message") and an opaque inbox identifier; your actual message stays in the app.
- Uploadcare (US) — storage and CDN delivery for profile and credential images you choose to upload.
Your rights
- Download a copy of the information we hold about you at
/api/me/data-export (APP 12). - Review every access of your information at
/api/me/access-log (APP 11). - Withdraw any consent below at any time from your account settings.
- Contact our Privacy Officer at privacy@elevateeap.com.au for correction or complaints. Unresolved complaints may be referred to the Office of the Australian Information Commissioner (OAIC).
